3D打印技术在种植牙患者中的应用价值探究(1)

     [摘要]目的:探究3D打印技术在种植牙患者中的应用。方法:研究对象选取笔者医院2017年1月-2018年12月收治的42例种植牙患者,共计128颗牙,使用随机数字表法将其分为常规组(20例,64颗牙)及3D组(22例,64颗牙),常规组使用传统种植牙技术进行手术,3D组使用3D打印种植导板进行手术。比较两组术后种植牙偏离距离、偏离角度,患者治疗前后咀嚼功能、成功率及满意度。结果:两组治疗前咬合力及咀嚼效率无显著性差异(P>0.05)。3D组术后种植牙颈部及尖部各方向偏离距离、近远中向及颊舌向偏离角度显著低于常规组(P<0.05);治疗后3D组咬合力及咀嚼效率显著优于常规组(P<0.05);3D组修复成功62颗(成功率96.88%),常规组修复成功54颗(成功率84.38%),3D组明显高于常规组(P<0.05);3D组满意度为94.45%,常规组为85.00%,3D组显著高于常规组(P<0.05)。结论:3D打印技术能有效降低种植牙患者种植体偏移,提高手术精度,提高患者咀嚼功能,改善患者满意度及种植成功率,具有临床推广意义。

    Abstract: Objective To explore the application value of 3D printing technology in implant patients. Methods A total of 42 dental implant patients with 128 teeth enrolled in our hospital from January 2017 to December 2018 were randomly divided into two groups (20 cases, 64 teeth) and 3D. Group (22 cases, 64 teeth).The conventional group used traditional implant technique for surgery, and the 3d group used 3d printed implant guides for surgery. The deviation distance and deviation angle of postoperative implants were compared between the two groups. The chewing function and success rate and satisfaction of patients before and after treatment were compared. Results There was no significant difference in the bite force and chewing efficiency between the two groups before treatment (P>0.05).In the 3D group, the deviation distance, the near-distal and the direction of the cheek and the tongue were significantly lower than that of the conventional group. The occlusal force and chewing efficiency of the 3D group were significantly better than those of the conventional group (P<0.05). In the 3d group, 62 teeth were successfully repaired (96.88%), while in the conventional group, 54 teeth were successfully repaired (84.38%). The success rate of the 3D group was significantly higher than that of the conventional group (P<0.05).The 3D satisfaction was 94.45% and 85.00% in the conventional group, and the 3D group was significantly higher than the conventional group (P<0.05). Conclusion 3D printing technology can effectively reduce the implant cost of implant patients, improve the accuracy of surgery, improve the chewing function of patients, improve patient satisfaction and success rate of planting, and has clinical significance.

    隨着社会的发展及人民生活水平的提高,人们对面部牙齿美观的认知及标准随之升高[1]。种植牙作为一种使用人工材料或装置对牙齿及其附属组织进行恢复的技术,其在临床应用也越广泛[2]。但在多年工作经验中,笔者发现多数患者合并多牙缺失且多伴有牙周缺失症状,在手术过程中,对最佳种植点的位置选取及种植牙准确植入角度的判定难度较大,传统手术导板效果不理想。近年来,随着三维数字成像及计算机技术的发展,3D打印技术出现并得到快速发展[3]。作为一种通过数据采集并进行三维重建的多层次连续打印技术,其打印出的3D模型具有高效率、高精度等优点[4-5]。笔者认为,若使用3D技术进行种植牙导板制作,对患者种植精度及种植牙效果的提高应具有积极影响,但目前相关临床研究较少,临床推广缺乏理论支持,笔者就此进行研究,近年来,对笔者医院就诊的种植牙患者使用3D打印技术制作手术导板进行治疗,疗效较佳,现将结果报道如下。, 百拇医药(张秀琴 刘振华 周俊宏)
